Evaluation of the relationship between vitamin D levels and related serum markers as well as disease activity in patients with rheumatoid arthritis: A cross-sectional study

Introduction: Many studies have reported the role of vitamin D in autoimmune disease including rheumatoid arthritis (RA) which is related to several serum autoantibodies such as RF and Anti CCP. It also has been shown that vitamin D deficiency affect the DAS28 and VAS score inversely. Therefore, the aim of this study was to assess the relationship between vitamin D deficiency and RA-related autoantibodies including Anti CCP and RF levels and also evaluate the association between these parameters and the severity of rheumatoid arthritis.Methods: In this cross-sectional study, RA patients over 16 years of age who refer to the clinic and rheumatology ward were enrolled. The severity of diseases was assessed via the DAS28 scoring system. Serum levels of 25(OH) vitamin D with other rheumatoid related laboratory evaluations including Anti CCP, RF and CRP were determined.Results: A total of 100 patients with a mean age of 45.27± 14.14 were included and evaluated; of them, 75% were female and 77% lived in the city. Most of the patients (66%) had moderate DAS28 levels; however, no substantial relationship was observed between DAS28 and vitamin D levels. A significant inverse relation between serum 25(OH)D level and disease duration, as well as a significant positive correlation with the level of education, was observed. There was also no significant correlation between RA-related autoantibodies (e.g. anti CCP, and RF) and inflammatory serum marker (e.g. CRP) with 25(OH)D.Conclusion: Due to vitamin deficiency in the majority of cases (73%) and the relationship between serum 25(OH)D levels and the duration of disease, it can be concluded that vitamin D levels should be checked in those patients.

Design and Fabrication of Mechanical Bed-system for Treatment of Adolescent Idiopathic Scoliosis.

Background: Scoliosis is a sideways curvature of the spine that occurs most often during the growth spurt just before puberty, which posses a postural disorder to individuals and also makes them walk with pain.Aim: the aim of this study is to design and develop a mechanical bed-system that will be used to conservatively mange adolescent scoliosis effectively and efficiently by reducing cobb angle and pain.Materials and Methods: materials needed are; Orthopaedic bed, Metal bars, Orthopaedic Foam, Fabrics, Metal pads, Metal bolts and nuts, Metal pipes. The methods followed are; fabrication of six-spring patient’s bed with the metal bars and then fixing six-spring foam on the bed. It was followed by fabrication/formation of metal pads for application of force on the patient’s spine, while lying down on the bed-system and then attachment of the foam pads to the metal pads. Three stands with a bolt and nut for attachment of the metal pads were fabricated and the components were fixed together.Result: the mechanical bed-system comprised of a metal bed, mattress, counter pressure pad, adjustable screws and metal stand.Discussion: although, bracing, casting and physiotherapeutic manipulations can reduce the cobb angle of scoliosis over a period of time (months), it is now believed that this device had given rise to a new technology that can stop and also reduce scoliosis curves more effectively and efficiently. Conclusion: The mechanical bed-system exerts more and tolerable pressure to the spine than the bracing, hence a better outcome.Keywords: Scoliosis, Adolescence, Treatment, Mechanical bed-system, Manipulation.

Comparison of Serologic Celiac Disease in Patients with Hypothyroidism and Healthy Controls: A Case-Control Study

Objectives: Celiac disease often accompanies by other endocrinologic diseases such as hypothyroidism. We aimed to determine the prevalence of celiac disease in autoimmune and non-autoimmune and clinical and subclinical hypothyroidism based on serological evaluation. Methods: This case-control study was conducted in a referral clinic of Kurdistan University of Medical Sciences, Sanandaj Iran. The case group was assigned as patients who had clinical and subclinical hypothyroidism (based on TSH levels), and the control group included patients without any thyroid or gastrointestinal problems. The levels of anti-tissue transglutaminase antibody (anti-TTG) were assessed in all case and control patients. Results: Two-hundred and fifty patients (150 in the control group) were included in the final analysis. The mean age of patients was 37.4 ± 10.4, and there were no significant differences for age and gender between the case and control groups (P-value > 0.05). The celiac serology was positive in five patients in the case group (5%) and two in the control group (1.3%). Celiac positive serology was significantly higher in the case group than the control group (P < 0.05). Conclusions: While undiagnosed and untreated CD causes comorbidities in hypothyroid patients, timely CD diagnosis and treatment diminishes the complications that were caused. Considering the significant relation and growing prevalence, it seems logical to design a screening program to detect CD in hypothyroid patients, especially the clinical ones with autoimmune hypothyroidism, especially those who require higher doses of levothyroxine.
